Thank you for choosing Alliance Obstetrics & Gynecology Group, LLC for your healthcare needs. We are honored that you have chosen to have your care with us. To assist in preparing you for your initial visit to our office, the following information is provided. Should you have any questions, please do not hesitate to call our office (407.960.2112). 


Pre-Registration Paperwork


There are several documents we need you to complete prior to being seen at Alliance Obstetrics & Gynecology Group. We utilize an electronic system known as Phresia to complete your registration and prepare you for your visit. After contacting our office and being registered as a patient, you will receive an email link to initiate this process. Please click on the link and provide all information requested as soon as possible. Completing these steps in advance will streamline your check-in process and allow more time for you to spend with your provider on your healthcare needs. 


If you do not receive the email link or are unable to complete the process prior to your arrival, please call our office for assistance or arrive 30 minutes prior to your scheduled appointment to allow ample time to complete everything in the office. 


What to Bring to Your First Visit


The following items will be required at your initial visit:


  • Driver’s license or photo ID
  • Insurance card
  • Form of payment (for copay and card on file requirement)
  • Referral form (if applicable)
  • Medical records (if any)


In preparation of your initial visit to Alliance Obstetrics & Gynecology Group, please contact your insurance company to ensure you are active in their system. 


If you are being referred by another provider/hospital or transferring from another practice, please arrange to have your medical records sent to our office at least one week prior to your scheduled appointment. Additionally, please obtain a copy for yourself and bring them to your visit to ensure a copy is available for review. Office visits which depend upon medical records which are not available at the time of the visit may result in the need to return to the office for a follow up visit or reschedule your visit entirely. If you are wanting to review test results or imaging which was performed at another facility, you must obtain those records and bring them with you to your appointment.
