There are several documents we need you to complete prior to being seen at Alliance Obstetrics & Gynecology Group. Completing these forms in advance will streamline your check-in process and allow more time for you to spend with your provider on your healthcare needs. Please print, complete, and return the new patient packet to our office prior to your arrival or bring them to your first visit. If you are unable to do so, please arrive 30 minutes prior to your scheduled appointment to allow ample time to complete them in the office. In preparation of your initial visit to Alliance Obstetrics & Gynecology Group, please contact your insurance company to ensure you are active in their system.
If you are being referred by another provider/hospital or transferring from another practice, please arrange to have your medical records sent to our office at least one week prior to your scheduled appointment. Additionally, please obtain a copy for yourself and bring them to your visit to ensure a copy is available for review. Office visits which depend upon medical records which are not available at the time of the visit may result in the need to return to the office for a follow up visit or reschedule your visit entirely. If you are wanting to review test results or imaging which was performed at another facility, you must obtain those records and bring them with you to your appointment.
