Pre-Registration Paperwork

There are several documents we need you to complete prior to being seen at Alliance Obstetrics & Gynecology Group. We utilize an electronic system known as Phresia to complete your registration and prepare you for your visit. After contacting our office and being registered as a patient, you will receive an email link to initiate this process. Please click on the link and provide all information requested as soon as possible. Completing these steps in advance will streamline your check-in process and allow more time for you to spend with your provider on your healthcare needs. 

If you do not receive the email link or are unable to complete the process prior to your arrival, please call our office for assistance or arrive 30 minutes prior to your scheduled appointment to allow ample time to complete everything in the office. 

What to Bring to Your First Visit

The following items will be required at your initial visit:

• Driver’s license or photo ID

• Form of payment (for copay and card on file requirement)
• ਰੈਫਰਲ ਫਾਰਮ (ਜੇ ਲਾਗੂ ਹੋਵੇ)
• Medical records (if any)

ਅਲਾਇੰਸ ਪ੍ਰਸੂਤੀ ਅਤੇ ਗਾਇਨੀਕੋਲੋਜੀ ਗਰੁੱਪ ਦੀ ਤੁਹਾਡੀ ਸ਼ੁਰੂਆਤੀ ਫੇਰੀ ਦੀ ਤਿਆਰੀ ਵਿੱਚ, ਕਿਰਪਾ ਕਰਕੇ ਇਹ ਯਕੀਨੀ ਬਣਾਉਣ ਲਈ ਆਪਣੀ ਬੀਮਾ ਕੰਪਨੀ ਨਾਲ ਸੰਪਰਕ ਕਰੋ ਕਿ ਤੁਸੀਂ ਉਹਨਾਂ ਦੇ ਸਿਸਟਮ ਵਿੱਚ ਸਰਗਰਮ ਹੋ।

ਜੇਕਰ ਤੁਹਾਨੂੰ ਕਿਸੇ ਹੋਰ ਪ੍ਰਦਾਤਾ/ਹਸਪਤਾਲ ਦੁਆਰਾ ਰੈਫਰ ਕੀਤਾ ਜਾ ਰਿਹਾ ਹੈ ਜਾਂ ਕਿਸੇ ਹੋਰ ਅਭਿਆਸ ਤੋਂ ਟ੍ਰਾਂਸਫਰ ਕੀਤਾ ਜਾ ਰਿਹਾ ਹੈ, ਤਾਂ ਕਿਰਪਾ ਕਰਕੇ ਆਪਣੀ ਨਿਯਤ ਮੁਲਾਕਾਤ ਤੋਂ ਘੱਟੋ-ਘੱਟ ਇੱਕ ਹਫ਼ਤਾ ਪਹਿਲਾਂ ਸਾਡੇ ਦਫ਼ਤਰ ਨੂੰ ਆਪਣੇ ਮੈਡੀਕਲ ਰਿਕਾਰਡ ਭੇਜਣ ਦਾ ਪ੍ਰਬੰਧ ਕਰੋ। ਇਸ ਤੋਂ ਇਲਾਵਾ, ਕਿਰਪਾ ਕਰਕੇ ਆਪਣੇ ਲਈ ਇੱਕ ਕਾਪੀ ਪ੍ਰਾਪਤ ਕਰੋ ਅਤੇ ਉਹਨਾਂ ਨੂੰ ਆਪਣੇ ਦੌਰੇ 'ਤੇ ਲਿਆਓ ਤਾਂ ਜੋ ਇਹ ਯਕੀਨੀ ਬਣਾਇਆ ਜਾ ਸਕੇ ਕਿ ਇੱਕ ਕਾਪੀ ਸਮੀਖਿਆ ਲਈ ਉਪਲਬਧ ਹੈ। ਦਫਤਰ ਦੇ ਦੌਰੇ ਜੋ ਡਾਕਟਰੀ ਰਿਕਾਰਡਾਂ 'ਤੇ ਨਿਰਭਰ ਕਰਦੇ ਹਨ ਜੋ ਦੌਰੇ ਦੇ ਸਮੇਂ ਉਪਲਬਧ ਨਹੀਂ ਹੁੰਦੇ ਹਨ, ਦੇ ਨਤੀਜੇ ਵਜੋਂ ਫਾਲੋ-ਅਪ ਮੁਲਾਕਾਤ ਲਈ ਦਫਤਰ ਵਾਪਸ ਜਾਣ ਦੀ ਜ਼ਰੂਰਤ ਹੋ ਸਕਦੀ ਹੈ ਜਾਂ ਤੁਹਾਡੀ ਫੇਰੀ ਨੂੰ ਪੂਰੀ ਤਰ੍ਹਾਂ ਨਾਲ ਦੁਬਾਰਾ ਤਹਿ ਕਰਨਾ ਪੈ ਸਕਦਾ ਹੈ। ਜੇਕਰ ਤੁਸੀਂ ਕਿਸੇ ਹੋਰ ਸਹੂਲਤ 'ਤੇ ਕੀਤੇ ਗਏ ਟੈਸਟ ਦੇ ਨਤੀਜਿਆਂ ਜਾਂ ਇਮੇਜਿੰਗ ਦੀ ਸਮੀਖਿਆ ਕਰਨਾ ਚਾਹੁੰਦੇ ਹੋ, ਤਾਂ ਤੁਹਾਨੂੰ ਉਹ ਰਿਕਾਰਡ ਪ੍ਰਾਪਤ ਕਰਨੇ ਚਾਹੀਦੇ ਹਨ ਅਤੇ ਉਹਨਾਂ ਨੂੰ ਆਪਣੀ ਮੁਲਾਕਾਤ 'ਤੇ ਆਪਣੇ ਨਾਲ ਲਿਆਉਣਾ ਚਾਹੀਦਾ ਹੈ। ਸਾਡੇ ਮੈਡੀਕਲ ਰਿਕਾਰਡ ਬੇਨਤੀ ਫਾਰਮ ਦੀ ਇੱਕ ਕਾਪੀ ਇੱਥੇ ਲੱਭੀ ਜਾ ਸਕਦੀ ਹੈ।
